Instrument forging there are 3 methods!
1) Cold forging is good for plastic handled kitchen scissors where the blades are usually 2 to 2.5mm thick. Anything thicker than this and foce must be applied. A nipper handle would 250-300 tons of force applied to cold steel, but only 20-30 tons in hot steel. Apart from the energy wasted, cold forging sets up small stress fractures when used in thicker steel. They are impossible to passivate and can harbour dirt and bacteria. Generally this is not the way to manufacture good quality surgical instruments. Cold forging however, good for 316 for trays, bowls, pitchers, etc.
Casting, again is not the process of choice for instruments since end result will be brittle and subject to damage if dropped on a hard surface like a floor. Casting does provide one advantage, its very exact & repeatable. For example, a item with a self-tightenir alignment joints are cast for accuracy.
So, its all about slection of steel,correct hardness, the process selected.
